Children's Hospital of Pittsburgh's website provides information for parents, kids, and teens in an easy-to-use format. www.chp.edu

From the American Academy of Family Physicians, this website includes information on common ailments, health tools such as BMI (body mass index) calculator and "Search by Symptom," over-the-counter medications, healthy living, and more. In the Parents & Kids section, special "kid-friendly" information is available. For example, information for parents on causes and treatment of vomiting includes, for kids, "What's puke?" www.familydoctor.org

This online service has existed for 10 years and provides a wide variety of medical information, including a Children's Health section found under "Healthy Living." www.mayoclinic.com

Annual Health Information

Every student must return a completed Annual Health Information form for the current school year to the Health Office. Return this form to school by your child's first day, even if exams are pending.

Medication Policy

This document outlines the current policy covering the handling and administration of all medications for students while at school

Medication Administration Form

The Medication Administration Form must accompany any medication to be administered to your child while at school.

Dental Examination Request

Please have this form completed and signed by your dentist if your child is entering kindergarten, grade 3 or grade 7, or if your child is new to Falk School.

Physical Examination Report

Please have this form completed and signed by your child's health-care provider if your child is entering kindergarten or grade 6 or if your child is new to Falk School.
